So I've noticed in people's AOS cover letters that it mentions "certificate of marriage" for the actual AOS documents, EAD, and AP... so that means we need THREE "Certified" copies for each or is one ok?

Also, since this is my first time actually working for a full-time job...I don't have a tax transcript. Will 4 pay stubs be sufficient? What else could I include?

Sorry, I'm new to the AOS portion, so please bare with me :)

I am currently in the United States under a valid K3 Visa Status. The place of my port of entry to the United States last May 03, 2008 was in Detroit, Michigan. My husband, _________, and I, _________, are filing an I-485 APPLICATION TO REGISTER PERMANENT RESIDENCE concurrently filing I-765 APPLICATION FOR EMPLOYMENT AUTHORIZATION.

Enclosed please find form I-485, Application to Adjust Status together with its supporting documents, and form I-765 with its own separate copies of supporting documents.


Form I-485 Packet and Supporting Documents:

1. Payment in the amount of $1010.00 personal check made payable to U.S. Department of Homeland Security.
2. Form I-485, signed and dated by _________.
3. Photocopy of Notice of Approval for I-130 Petition.
4. Medical Vaccination Form 3025 and Supplement Form I-693 sealed envelope from Civil Surgeon.
5. G-325A form, Biographic Information Sheet (4 pages).
6. 2 Passport Size/Visa Photos with name and Alien Receipt # printed at the back.
7. Copy of Birth Certificate.
8. Copy of Passport Biographical Page and K3 Visa Page.
9. Copy of Form I-94 (front and back)
10. Copy of Marriage Certificate


12. Notarized Form I-864, Affidavit of Support – signed and dated by Petitioner

12.1 Certificate of employment
12.2 6 most recent pay stubs
12.3 Tax transcripts from 2005, 2006, 2007
12.4 W2’s from 2007
12.5 1040 IRS Form filed Year 2007
12.6 Most recent bank statement
12.7 Biographic page of Passport
12.8 Copy of Birth Certificate


Form I-765 Packet and Supporting Documents: (Application for Employment Authorization)

1. Copy of Passport Biographical Page and K3 Visa Page
2. Copy of Form I-194 (front and back)
3. Copy of Marriage Certificate
4. 2 Passport/Visa size photos with name and Alien Receipt # printed at the back.


Copies of documents submitted are exact photocopies of unaltered documents and I understand that I may be requested to submit original documents to an Immigration or Consular officer if requested.

You should have one certified coy for each form as they go to different places. Get a letter from your employer as to annual salary and length of employment and that the plan on keeping you.

Thank you for your quick response and useful information! :) So you have to buy 3 extra copies just for the AOS package?!

One other quick question: On the I-485 the guide states to use my spouse's new married name (my last name) and to list previous names. On the form it has no specific space to list previous names... it it necessary?

From the I-864 instructions>

If you were required to file a Federal income tax return during any of the previous three tax years but did not do so, you
must file all late returns with the IRS and attach an IRS-generated tax return transcript documenting your late filing before
submitting Form I-864. If you were not required to file a Federal income tax return under U.S. tax law because your
income was too low, attach a typed or printed explanation.
If you were not required to file a Federal income tax return
under U.S. tax law for any other reason, attach a typed or printed explanation including evidence of the exemption and
how you are subject to it. Residence outside of the United States does not exempt U.S. citizens or lawful permanent
residents from filing a U.S. Federal income tax return.

As to the marriage certificate, you keep the certified copy you purchased from the court and just make copies of it to send with the forms. Only a copy of it is required.

It asks for other names used on the G-325A and I-765.
