Get the embassy letter (http://photos.state.gov/libraries/manila/1178798/feb2015/K1-K2%20Applicant%20Interview%20Preparation%20Instructions%20-%20English%20_February%202015_.pdf)and follow it to the T for all needed documents. Nothing more and nothing less and you will glide right through the interview. Here is what the EL says about financial documents:

EVIDENCE OF SUPPORT. You should be able to demonstrate that you will not become a public charge or burden on the U.S. taxpayers for financial support. A completed I-134 Affidavit of Support Form may be useful to the consular officer to evaluate your petitioner’s ability to be financially responsible for you. You may also be asked to submit your petitioner’s most recent U.S. Federal income tax return (Form 1040) and wage statements (Form W-2). Employment letters stating salaries and bank statements may be included to substantiate the I-134. Download Form I-134 at http://www.uscis.gov/portal/site/uscis.

No. Just present it at your interview and if there are any questions from the CO during the interview, just say opps and answer that you had made a mistake. This is not a big deal as other before you have don't the same and were approved.

No you do not need to complete a new DS-160, the travel date you listed on that form means nothing.

Follow the embassy instructions letter for what is required. It states the most recent tax return and W-2 are required (not three years of returns, so just 2014). You don't need bank statements. But you do need a copy of the W-2, have your fiance email a copy to you.
