My fiance just completed his medical exam with Dr. Dennis in Bogota, Colombia.

The doctor told him that vaccines are completely voluntary because he will be a K-1 visa holder and he can complete them in the US.

My fiance has his vaccine records and all he needed was the MMR, so he got that at the medical.

My question is, what are we going to do when it comes time to AOS and the I-693. If the doctor said its voluntary I am scared he won't complete something the correct way.

When it is time for the AOS do we complete the I-693 ourselves since he has his vaccine records? Or will the embassy give us a copy from the sealed envelope?

I'm sorry if my questions sound confusing, I didn't even know how to word this correctly. My fiance is sitting at the doctors office right now waiting for the sealed report. So I was hoping someone could answer me so I can call him and tell him.

I guess what I should have asked is where will my fiance get the DS-3025?

The doctor told him he cannot give him copies of anything. But when you look at the DS-3025 example online it says in capital letters "GIVE COPY TO APPLICANT".

Will he get this at the interview or should he demand one from the doctor?

The doctor told him that the K1 is a NONimmigrant visa and the DS-3025 is not required for the K1 so he is not even filling it out.

Medical examination is valid for one year (and it is included in the yellow envelope that your fiance will give to the immigration officer at POE) what your you need to do is once he arrives in the States see a civil surgeon and ask him to complete the I 693 Vaccination Report (bring the immunization card from Colombia) the Dr will give him a sealed envelope that needs to be attached to the I485 form.

I recommend to call a few of them and ask how much will cost to transcribe the immunizations.

Apparently complete immunizations are not a requirement for nonimmigrant visas, Dr Dennis said the date of my appointment that since May last year they are not obligated to fill the DS 3025 because vaccinations are mandatory for adjustment of status or immigrant visas only and the embassy do not ask for them in the package of instructions, he only give me 2 CDs with the results of our lung x rays.

I have seen k1 visa holders from other countries get that form after their medical, that's why I am planning to include a note on my cover letter for AOS that states I got all the required vaccinations prior my entry to the US and the form DS 3025 was not given to me by the designated physician at the moment of my medical examination and send the I 693 sealed envelope filled by a civil surgeon we contacted already, he will charge US$100 for both of us.

"....Even if a new medical examination is not required, you still must show proof that you have complied with the vaccination requirements. If the vaccination record (DS 3025) was not properly completed and included as part of the original, overseas medical examination report, you will have to have the vaccination report completed by a designated civil surgeon. In this case, you are required to submit Part 1, Information About You, Part 2, the vaccination chart, and Part 5, the Civil Surgeon's Certification, of Form I-693 (in an envelop sealed by the civil surgeon). Please see the instructions for Form I-693 for further information.

09-28-2007 = NOA1, 23 days after their 45-day promise to send it (grrrr).

10-20 & 11-14-2007 = Phoned ImmOffs; "still pending."

12-11-2007 = 180 days; file is "between workstations, may be early Jan."; touches 12/11 & 12/12.

05-05-2008 = Infuriating $12 call to non-English-speaking consulate appointment-setter.

05-06-2008 = Better $12 call to English-speaker; "joint" interview date 6/30/08 (my selection).

06-30-2008 = Stokes Interrogations w/Ecuadorian (not USC); "wait 2 weeks; we'll mail her."

07-2008 = Daily calls to DOS: "currently processing"; 8/05 = Phoned consulate, got Section Chief; wrote him.

08-07-08 = E-mail from consulate, promising to issue visa "as soon as we get her passport" (on 8/12, per DHL).

08-27-08 = Phoned consulate (they "couldn't find" our file); visa DHL'd 8/28; in hand 9/1; through POE on 10/9 with NO hassles(!).

If the panel physician gave him the MMR vaccine at the K-1 medical, then he needed to fill out the DS-3025 to show the record of him receiving that vaccination at least. Get proof of that vaccine. You can take it with you to a civil surgeon in the US.

Mexico doesn't give K-1 applicants vaccinations either, unless things have changed recently. My husband received a useless, blank DS-3025.

Is not just him, in the package of instructions there are 4 authorized physicians and they all said the same, since last year they are not obligated to fill the ds 3025 form.

I just got off the phone with the embassy informing them and asking what to do. They said "you need to send an email to IVbogota@state.gov for further assistance."

Completely unhelpful. I emailed them 3 months ago about something and they never replied to me. Our interview is in exactly 1 week.

What should I do? Do you have any advice?

Vaccinations are not required for the K1 and the doctor isn't required to give you a DS 3025 either, however, the beneficiary will have to see a civil surgeon in the US to get all vaccinations transcribed to the vacciantion page of I-693. My fiancee at the time got a DS 3025 but it was blank, useless.

Note that the whole medical exam does not have to be performed, but some civil surgeons will not do the vaccinations part only, most want to perform the whole medical exam again.

Did you have the I-693 completed to fix this problem?

The vaccinations have nothing to do with entering the US, so you won't be denied at the POE. You will give them the sealed envelope when entering. Your medical exam documents will be in there and everything in that envelope will be sent to the USCIS and placed in your case file.

The DS-3025 is not a requirement for AOS. They vaccinations and proof of them is. If you didn't get them overseas, then you need to get the in the US. You will need to see a civil surgeon in the US for an I-693, as my husband did. You do not need the medical again. They should give you a copy of the I-693 and then seal the original in an envelope for you to include with your AOS paperwork. The link I posted above shows the I-693 instructions and what you need if you had the K medical already. You can read through the entire I-693 instructions. It gives a lot of details.

(2) Even if a new medical examination is not required, you must still show proof that you complied with the vaccination requirements. If the vaccination record (DS 3025) was not properly completed and included as part of the original overseas medical examination report, you will have to have the Part 7. Vaccination Record completed by a designated civil surgeon. In this case, you must submit Parts 1., 2., 4., and 7. of Form I-693.

Finding a civil surgeon that will not insist on doing another complete medical can be difficult. You will need to call around.
