Hello everyone,

On the G-325a it ask to state my last five years of employment.

I have two questions, the first is that for a two year period of time I was self employed as a maintenance worker for a small company and only worked a day or so a week. I have gaps in time where I haven't done anything for them for 2 or 3 months. (at the exact same time I had another typical part time job (not self employed). I have no Idea which months I did and did not do any maintenance work. Would it be okay to say I was self employed durring the entire 2 year period or should I show the gaps in time?

I was also wondering what should I put as the adress for the self employed job. My home adress or the adress for the company I did the maintenance work for?

Yes, you can list the self-employment for the entire 2 year time span. You do not need an address. You just put Self-Employed and list the occupation, maintenance worker.
