is it okay to go and do my rom without my husband? since i already have all the requirements?

and what format for parents full name do i need to do?

last question?

what last name do i need to use in rom is it my new last name or my old last name?

Please only ask this question in one thread and not multiples. Also, you are in a thread that is more that a year old. Your better off starting your own thread and ask the question. But right now, your question is similar in the other thread that you posted in. Take a look please.

The correct format is First, Maiden Family Surname, Husbands Surname. Use it for all your new documents. Stick with one.

What do you mean by - without your husband? The form must be signed by the husband and the wife, plus it needs to be notarized if you are mailing it.

As for your name, the form is completed in your MAIDEN name

Procedure and Requirements:

1. Four (4) original duly accomplished Report of Marriage Contracted Abroad form, typed or printed legibly in black or blue ink, signed by both husband and wife. Wives should indicate their maiden name as the last name. If submitting by mail, the complete forms must be notarized.

2. Covering letter (handwritten or typed), address to the Consular Section, indicating service/transaction requested and telephone number or email address for contact details.

3. One (1) original or certified true copy and four (4) photocopies of the Marriage Certificate or Contract (original will be returned).

4. One (1) original or certified true copy and four (4) photocopies of the Birth Certificate of the husband and the wife. The Consular Officer reserves the right to require additional proof or documents from an applicant, to prove his/her citizenship or identify pursuant to the Philippine Passport Law (R.A. 8239) and the Foreign Service Act (R.A. 7157).

5. Four (4) photocopies of a government-issued identification card or passport of both husband and wife.

6. Four (4) photocopies of Philippine birth certificate of Filipino spouse.
