Hey! So I'm in the process of assembling all the paperwork needed for my AoS package and I'm not entirely sure what to do when it comes to seeing a civil surgeon. So I had my medical done in Sweden this June and the doctor there ticked the vaccination waiver box so I didn't need to get any additional vaccines. Upon my PoE I only had one huge 'do not open' envelope to hand over to the immigration officer and they kept everything that was in it so I don't have my DS-3025 form. From what I understand, if the vaccination waiver box is ticked then I'm good to go without any additional vaccines? Does this mean I don't have to see a civil surgeon and can skip the I-693 entirely? Thanks in advance!

I already checked it but it didn't really answer my question. But I guess I'm good to go without the I-693 then if my doctor didn't mention needing any vaccinations once I've arrived in the States.

If your PROPERLY filled out DS-3025 is in the sealed envelope you will be good to go. If your AOS drags on until over one year from filing, (as ours did) you will be required to file a updated I-695.

The info about what vaccinations were needed and how the form is filled out, plus this part of that thread didn't answer your concerns? >

What if I don't have a DS-3025?

If you are positive that you got the needed shots, then actually mailing in a DS-3025 photocopy is not necessary. The original form is with your other medical results and was turned over to USCIS at POE. That's actually the one that counts because it remained in the "chain of custody" so you didn't have an opportunity to alter it. The photocopy isn't really an official copy and is more for a reminder that you got complete at your visa exam. Of course without having a copy, you don't have the opportunity to see if the rest of the form was completed correctly.

Not really, it basically says I gotta look at my DS-3205 to determine whether I need an I-693 but since I don't have a DS-3205 and the only time I saw parts of it was when filling out the tick boxes for whatever medical conditions I have I can't really know what 'filled out properly' means... since I never saw the entirety of it. I'd rather make sure that I understand the information correctly than just wing it and get an RFE back and further delay the whole process, y'know?

If you want to make sure, then you need to get a civil surgeon to do the vaccinations portion of an I-693 for you. You will need to find one that won't insist on doing a whole new medical that you don't require, unless you are ok with paying and repeating the medical too.
