Hello everyone,

I'm the USC and filling out my G-325a for myself and had a few questions regarding employment over the past five years.

First off I've been self employed periodically since August of 2012 for several different people. Twice as a contractor working for a business and at the moment working for an individual building a house. Should I state, something to the affect as "Self Employed Laborer"?

Also should I list my home adress of the adress of the people/business that was paying me at that time?

My next question is regards to the time line. From August of 2012 to August of 2014 I worked periodically for the same business, one of the two I was self employed by. over those two years I sometimes worked one or two days a week and but then I have some breaks where I might have not done any work for them for 3 months at a time. Do I list these gaps in time? (I know the easy thing is to just put it on the application, the reason I'm asking is because I can only remember "rough" dates and I'm not use if I'm off by a month if it'll cause any problems.

I also worked for a Company(not self employed/ received W2) for a period of roughly 4 months from august of 2014 to November of 2014. However there was a gap durring that time that for about a month/ month and a half when I did not work and was unemployed. I don't know the exact dates of this either. Weather it was from late august to september or september to october. Etc.

From January to may of 2014 I worked for a Paint and Body shop and also received a W2 working there. However my boss had a side business that I'd work in to make extra hours where he paid me with a 1099. I made about 700 dollars in this. This was just from one job that we had at the very begging of may I believe. So should I just list the month of may (even thought it was only for about a week)?

Last question, in the summer of 2010 and 2011 I worked for a nonprofit summer camp I probably made under $1000 each summer. I was told it wasn't enough to worry about filing so I didn't file.(I received a W2 for each summer). Would it have caused any problems with USCS since I didn't file?

One more question, sorry. Over the past five years I've done some work for my church/private school. maybe two or three times a year and have never made more than 300 or 400 dollars. I have no Idea of any of those dates. Just really rough guesses.

Sorry for such a long post and if it's hard to understand. I've thought about making a very detailed table like the one on the G-325a on a separate sheet of paper and listing everything and putting asterisks by the dates I might be off on and explain below that I could be off by a month or so. But I'm not sure if that would be a good or bad idea.
