I've read several posts about this but still am very confused about our RFE and what we need to do next.

We recieved a RFE that said "You submitted a vaccination record as evidence for your medical examination. You must submit a complete medical examination report either by getting another examination and using Form I-693. Report of medical Examination and Vaccination Record, or send the complete medical examination report which was part of the visa packet given to you for entry into the United States."

It's been 6 months since I've looked at any of our paperwork and I pulled it when we got the RFE in. We have my husband's vaccination records from his family doctor showing his shots given to him as a child and then we have the certificate of immunization from Knightsbridge Doctors where he got his medical, along with his reciept for paying for his medical at Knightsbridge. I also found a CD he recieved at his medical that we haven't opened. Is that something I need to get documents off of to send back with this RFE?

I specifically made sure he got all the necessary shots he needed before he came over here so we wouldn't have to have him do another medical. What else do we need to do?

Best bet will be to get a cmplete I-693 (medical exam) done by a civil surgeon, that will also document all vaccinations required for AOS. They are supposed to have the medical exam in their system but seems like there is an issue somewhere.

If the DS3025 was not completed, which is sometimes the case, at the medical exam at home country than it needs to be transcribed to I-693 by a civil surgeon.

The CD only contains x-rays and that is for your records only.

I'm not sure to be honest. Do you think I should send that in? Or just go civil surgeon? This is what I have:

That is the record of the shots given but not the vaccinations record form, it should have been Form DS 3025 listing all vaccinations required for the beneficiary. It it was given incomplete or not given at all, you need to see a civil surgeon and complete the vaccinations part of the I-693. Be aware though, most civil surgeons want to do the whole medical exam even though is not required.

Okay thank you. I don't see that in any of records. Is it possible he came here without that form? And how do I get the civil surgeon to fill out the form for me without giving us the medical he doesn't need? Everytime I call an office they say it'll be around $145 for the immigration medical.

That is the issue with most civil surgeons, they want to do the whole medical exam and not just the vaccinations page of the I-693.

OP: Did you send in the form DS-3025 - the one that Knightsbridge should have given at the medical exam? Looking at the document you uploaded only shows they received some shots during the visit, it's not the 3025, which shows a list of vaccination history marked with checkmarks and letter codes. The fiancé should have received this document at their medical.

I asked my guy if he has the DS-3025 last night, but he said he thinks the embassy kept it. I'll be looking for it after he comes here.

It seems you would have to go with one of the above solutions of going to the civil surgeon, if you can't find the DS-3025.

Make sure he takes all the records with him so they don't try and give him the shot's again, you will find they will try and say he still needs one thing or the other looking to bill you for the vaccination.

Also another note, if you just received the documents you could try contacting the embassy and finding out if they could forward it or something like that, that's if you still have time, before the time on the RFE expires. (Just a suggestion, not suggesting its been done before, before people start crucifying me for saying so)
