6. A copy of T's passport page with Non-immigrant Visa This needs to be a copy of the bio page. A copy of the K-1 visa is not needed.

7. 2 T's passport-style pictures

8. A copy of I-94 Not needed.

You may also wish to consider sending in the I-131 application, because it's quick and free! Even if you do not plan on using the AP, it is still better to have it and not need it than for an emergency to happen and you don't have it.

For filing AP, all you need is:

  1. Form I-131
  2. Copy of T's passport bio page
  3. Copy of T's K-1 visa
  4. 2 T's passport-style photos

Some people have been receiving RFEs for proof of legal name with the I-765 and I-131, so it is not a bad idea to include a copy of the marriage certificate with those 2 forms, even though the instructions do not specifically ask for it. http://www.visajourney.com/forums/topic/563940-concurrent-filing-of-i-485-i-131-and-i-765-attachments/#entry7796074

Item #10 - Is that a copy of DS-3025 (Vaccination Worksheet)?

Yes ,that right frontgear ....On those form do I need fill the Alien Registration (A number) of the principal Immigrant ,and that A number I can see on I -797 above her name ? Thank frontgear

You do want to put the A-number on all applicable forms. It only helps.

For I-765 (EAD), make sure:

1. Put [c][9] on question #16

Also, if you fill out the I-131 (AP), make sure of the following:

1. Part 3: Put UNKNOWN in Date of Intended Departure and Expected Length of Trip (in days)

2. Part 7: Check More than one trip box

i only submitted 2 passport style photos for i485. i765 and i131 i did not submit but u could play it safe and include addl passport style photos.

I played it safe and sent 6 passport style photos, but I did it for cheap. I uploaded my wife's passport photo that we used for the K-1 to an online site and copied it 6 times onto a 4x6 format that I had developed at WALMART in 1 hour for $.09. The photos came out great and I didn't have to pay the $7.96 WALMART charges or $12.99 that WALGREEN'S charges for two 2x2 passport photos. All I had to do was cut them out!

2015-01-04 :Submitted I-129F to Lewisville, TX Lockbox via USPS

2015-01-06 :Received delivery confirmation from USPS and routed to the USCIS California Service Center

2015-01-08 :NOA-1 (Received by text and email)

I'm sure we'll be just fine with the PP photos we used. The last PP photos she took was for her medical exam which was in March and I submitted our AOS well before the 6 month mark. Thanks for your concern, but I'm pretty meticulous when it comes to filing this paperwork. I check 5 or 6 times, before I submit anything and it has worked for me thus far.
