No he does not have that paper. Unfortunately, they didn't give him any of that.

Well maybe you can include the instruction of the i485 which says that adjusting from k1 doesnt need the i693

Hi everyone!

My husband came here in August on the K-1 visa. We have since gotten married!! We are currently getting the AOS package ready to send in, but I have been reading many forums and have been unable to conclude a concrete answer on his vaccination records. He did have his physical overseas and I know they put the vaccination records in the sealed envelope he left at immigration. I am now trying to figure out what we should send in as far as the i693 form. Do we just need to fill out part of the i693 per directions for K1 visa holders? Does he need to go to a civil surgeon to have the DS-3025 filled out? What did you guys send in regarding that? Any help is greatly appreciated! Thank you!!

I had my medical exam done for k1 visa overseas( X-ray & blood test), they did not give me any copy of DS-3025 , which i think it was included in the no-open package i gave at the PO.

As i have no medical record , i had to take the shots again and get the form I693 completed by a civil surgeon nearby.

I applied for Aos and waiting for bio appt, Hope that would help .

That definitely does help! We're going to call and see if they have a copy of the forms they filled out. Keeping our fingers crossed that they do! He has an immunization record booklet that the doctors filled out so maybe that will work? It's not the DS-3025 form, but maybe it could work.

Does your husband have his vaccination card? All he needs to do is to fill out the i693 and take it to civil surgeon to transfer the vaccinations to the i693 form and seal it. I shouldn't cost that much. Unless some vaccines are not recorded in the vaccination card.

Hello, your husband only needs to get a civil surgeon who works with USCIS to fill out and sign the form I-693 with his vaccinations. The doctor will tell him if he needs more shots according to his age and health. It is very cheap. Mi doctor only charged $20 . Don't send his AOS without this doc...he could receive a RFE.

As I mentioned earlier, We went to the uscis local,office in York, PA with the biometric appointment letter, we asked if we could do them right then, instead of our appointment on Friday. They said yes. We arrived at 12:30 and left at 12:45. Thanks to those vjers who advised to try a walk in.

So we got our paper NOA1for I-765, but not for the other two forms. We never got emails or texts even though we sent in the form for that. We do have a different mailing address than physical address,that we can not receive mail at, so I am concerned that maybe the other two were sent to our physical address for some reason. :/

They normally send it out at the same time though. You may need to call them a month after your notice date (they should have the same notice date) if you havent received anything in the mail.
