The airline will probably give you that one. It is simply your name, address and occupation. "Homemaker" is an ok answer for occupation if you do not have a specific occupation. You will give that form to Philippines immigration along with your passport at the airport so that you can exit the Philippines.

Make sure your petitioner gives you your address in the USA and that you memorize it or write it down in a safe place to include house number, street address, city, state and postal (zip) code. For example:

123 Main Street

Salem OR

98765

Before the airplane lands in the USA, you will be given additional SHORT forms to fill out with similar information as the Philippines immigration form. There will be yes/no questions on the form as well. All your answers will most likely be "no".

I believe that was a default message that he is used to send to whoever asks pretty much questions on POE :P

Yup your right. :) In my haste I copied a post that I use over and over to answer this same question but normally it's used in the Philippines sub-forum and therefore I neglected to change the references to the country. My bad.

Just the same everything is still the same whether from Timbucktoo (it's a real city in Mali) or wherever.

I received packet 4 by e-mail this morning (I'm the petitioner) and well the Paris consulate gave us an interview for February 2nd ... I'm kinda bummed, I was hoping we could pick it ourselves for next month. Everything has been moving so quickly, and now we get an interview 2 months away ...

I wonder if we can change it? ... once we pay for the visa we can schedule the interview, and I saw that they had available as soon as next week! But the e-mail says only that day (Feb 2) is valid.

Ours says ready as of Nov 23. Maybe because of the APEC. I'm not sure. Lol But we scheduled our interview on Dec 10, to give us enough time for the papers. I hope you'll get also a ready status.
