Hi Angel291984,

For our CRBA, we had the appointment on December 28, 2015 and we got the packet (CRBA and passport) on January 18, 2016. No additional documents asked.

You can also track your packet via Air21.

Hope this helps.

Hello im a newbie here and will apply CRBA for my 5 mos old son this March. Is it ok to file without your US Citizen husband? Does he need to file additional requirement like DS - 3053? We read the updated checklist online and it said that the absent parent must sign the DS 3053 and get it notarized ? Hope you could help me with this

Statement of Consent (DS-3053) is required for passport applications for children under the age of 16 if only one

parent is appearing for the interview. The absent parent must sign the DS-3053 and get it notarized by a U.S. commissioned

notary. The document is valid for 90 days after the date of signature. If only a legal guardian will accompany the child, a DS-

3053 from both parents is required. Download this form at http://photos.state.gov/libraries/manila/19452/pdfs/DS-

3053_Jan2013.pdf"

DS-3053 is required if you are applying for the passport and only one parent is present. Its not a requirement if you are only applying for the CRBA.

Hope this helps :). But yeah its better to apply CRBA and passport together

I am gathering all the paperwork for my daughter's crba and US passport. Both parents are USC but only me is applying for both documents. With the Statement of Consent, do I need to send this to my husband in the US and have him sign and notarized there? or can I have it notarized here. Please advice.

Thanks

notary. - Just need to clarify who are they referring to as U.S commissioned notary.
