I searched and searched and there appears to be no clear answer...... So......

My wife and step-daughter are here on a K1 and we are filing their AOS. Looking at the vaccines provided in Manila at SLEC, they received the following vaccines:

Wife:

DTAP

MMR

Varicella

Influenza

Step-daughter:

DT,DTP,DTAP

IPV

MMR

HEP B

Varicella

Influenza

There were no blanket waivers listed and the other lines are blank. Are we good to go with just the DS-3025 or do they need additional vaccines?

Clear answer: The I-485 instructions (page 4 section 7c) state to include a copy of the vaccination record (DS-3025). I always suggest you print out a copy of page 4, highlight section 7c and attach it to the copy of the DS-3025 as many adjudicators don't know the actual requirements, those that have done it this way have had no issues. I have contacted USCIS on this very topic and they confirmed what I have posted.
