St. Luke's physical and document requirements

The documentary requirements for the physical at St. Luke's and return visit to pick up the results are almost duplicates.

This is very overwhelming and making me tired again....

Here is the list for the physical:

What to bring:

  1. Bring passport + 2 copies of passport bio page

  2. 2 copies of NVC letter/email when MNL# is assigned (SLEC requires the NVC letter/email and the appointment confirmation letter from the embassy that lists the beneficiary's name & the MNL #).

  3. Three 2X2 passport photos.

  4. Copy of annulment is asked for and a psychological evaluation could be required if psychological incapacity is listed as the reason in the annulment. (She has never been married so this is moot.)

  5. 2 copies of the DS-160 <-- this must be the printed confirmation page for submitting the DS-160?

Then the requirements to pick up the results:

When returning to SLEC (for results/vaccinations) bring the following:

  • Passport

  • 2 visa photos <-- this is not available until St. Luke's and interview are completed. What do they mean???

  • Appointment letter/confirmation page (original & 1 photocopy, or 2 copies of email or page from website)

  • SLEC official receipt.

  • DS-160 confirmation page

These are documents that are required for both your fiancé and K-2 children for their medical.

EACH APPLICANT MUST PRESENT THE FOLLOWING:

a. Valid passport

b. 2 photocopies of Passport Biographic/Data Page, the page in the passport that contains the applicant's photo and information. Photocopy on short bond paper. Passport photocopy should be the same size as the original, not enlarged or reduced.

c. 4 pieces recent 2"x2" visa photos (see visa photo guide), 3 for submission to the U.S. Embassy, 1 for clinic file. ALL PHOTOS MUST BE IDENTICAL. Write the applicant's complete name at the back of each photo.

The US Embassy requires you to bring the 2x2 photos for your visa issuance. We regret not to release the medical report until such time that you can provide this requirement.

d. Letter of Case Number

◾Principal Applicant - original & 2 photocopies

◾Derivative - 2 photocopies

If Letter of Case Number was received via email, print 3 copies for the principal applicant and 2 copies for each derivative. If you have not received the email from NVC with your case number (MNL#) then provide they a copy of the interview confirmation letter/email/webpage printout, OR the NVC letter OR a screen print from CEAC with your MNL#. You'll need one of these letters confirming your MNL#.

e. Visa Interview Appointment Confirmation from the U.S. Embassy , if available

◾Principal Applicant - original & 2 photocopies

◾Derivative - 2 photocopies

If Visa Interview Appointment Confirmation was received via email, print 3 copies for the principal applicant and 2 copies for each derivative.

f. For applicants who registered online, 2 copies of the online registration confirmation form

a. Applicants who did not register online are required to provide the name, address (include city, state, and zip code), contact number, and email address of their petitioner in the U.S.A.

◦Minors (17 years old and below) must be accompanied by their parent or by a legal guardian who has thorough knowledge of the child's medical history.

In case the companion is not the legal guardian, a special power of attorney or authorization letter from the petitioner stating that the companion is authorized to sign on his behalf for the applicant's medical examination and result, must be presented. The companion must also present a government-issued ID as proof of identity.

◦Regardless of current marital status, applicants who have had previous marriages that were nullified through annulment or terminated through divorce are required to present the annulment or divorce papers (court decision) before their medical report can be completed.

Do you have a email from NVC that contains your MNL#? If not then go to CEAC and print off a screen shot from there. That's all that you will need to complete your medical.
