Hello, does anybody have a copy of an I-693A form, or a link where I can see a copy of this form. That way I can see the vaccinations that are needed for Adjustment of Status. As I mentioned, my fiancee lost her DS-3025 form, so I need to know what vaccinations she needs to get again. Thank you.

Please don't let her repeat the vaccines all she needs is to go too a lab or Drs office and get her titers drawn.

MMR titer= Measles Mumos and Rubella

Varicella titer= chicken pox

Hept b titers

She may need a tetanus if she has not had one in the last ten years.

If she didn't get a x-ray it will be safe to get a PPD if she has never had a positive PPD before or if her country did not vaccinate her with the GHB vaccine then she will have to have another xray

Congrats you found the form. However it still needs to be filled out by a Doctor so what good does it do you?

you need to take your ds-3025 (should have that from the medical exam in country of origin) to a uscis certified civil surgeon, who will fill out the i-693a. if you take it to a doctor who is not uscis certifed as a civil surgeon, you're wasting your time and money. you will get one copy in a sealed envelope to submit with your aos packet, another copy for you just in case uscis loses the first. here's the site to locate a civil surgeon. note that the cost of transcribing the ds-3025 will range around $25 to upwards of $50 or more.

So, I was told I could get this done when i get to the states. Now my question is .... I don't have form I-693A - only DS-3025. Thank you! I made an appointment with my DR to get the other shot. I also called that DR in Austin that transfers everything to the I-693A form for only $10. I got discouraged at first because I called civil surgeons in my area and they all wanted to charge an arm and a leg AND do a complete medical exam again. Nobody wanted to just give me the missing shot and fill out I-693A. Thank god for this forum cuz after searching forever, I found that DR in Austin. :-)

Not strange at all. Look carefully at the form at the link you reference and tell us what form # you see on the form?

I-693A is a 'form #' created by people's imagination (as is 'NOA1), it is not the vaccination supplement's form number.

Wow, 10 bucks! That is about the best I have ever heard of. I thought we got off cheap for $70 that included her tetanus shot.

Lets just say after paying almost $4 a gallon for gas, I am not too eager about shelling out another $400 if I don't have to.

The one year clock is from date of medical exam to date of I-485 application. If you are less than one year then you are good to go, as they will pull the exam from the permanent immigration file to use.

Your only addeed requirement then is the vaccinations.
