Hello. I'm afraid my fiancee has lost her DS-3025 form (don't ask how), and I called her doctor in the country she got her medical exam and he doesn't have a copy of it either. She has no records of her vaccinations, except what was on that form. So now my question is, can she simply get all those vaccinations done here again, even though she just got them? Is this risky for her health. And if we go to a civil surgeon, does he need to see the DS-3025 form, or can he transcribe the vaccinations over from a regular vaccionation booklet? Please advice, this is worrying us a lot. Thank you

ok you will either have to get the medical again or you could get a blood titre test that will show all the vaccinations you have had(my hubby just did that b/c he did not have a copy of the vaccination history or the ds-3025).

once the testing is complete-you can take the results to a gov't approved doctor & they will fill out the I-693.

I hope I explained this correctly-someone else will clarify.

Does anyone else think that we should have a separate forum for vaccinations?? It seems that there are a ton of questions about this everyday(I am guilty of asking questions all the time :whistle: )

We were able to have the civil surgeon transfer the vaccinations from the booklet because we did not have the ds-3025.

03-20-06 - Sent to CSC

03-21-06 - Receipt at CSC by FEDEX

04-06-06 - Officially received by CSC

04-11-06 - Check cashed - received case number

07-03-06 - Received IMBRA RFE

07-05-06 - Sent RFE reply over night

07-06-06 - USPS receipt notification

07-12-06 - Received confirmation of receipt from USCIS

07-13-06 - Touched

08-01-06 - Received email Petition Approved

08-05-06 - Received approval via snail mail

Thanks irishgirl73. Just a couple of questions. Do you remember what vaccinations/titers your husband got for the I-693? That way we can go to a local health clinic and ask that she get those vaccinations specifically. Also, my fiancee told me she got blood tests done for her medical exam in Venezuela, that tested her for HIV and some other disease I believe. Does that also need to be indicated on form I-693A? Does she need to get another HIV test or does the I-693A just deal with vaccinations? Thank you.
