my hasband only has his imunization card....didn't get anything else from the Dr. just the reciept. what is this ds-3025 and if we don't have do we need it and how would i go about getting it???

you should be able to take your vaccination history to a gov't approved doc, and they will fill out the I-163 that you need.

I do not have the link for the doctors, but I know I saw it in this forum somewhere. Maybe someone else will post it for you.

My husband used his immunization card from Yard to get the I-693A filled out and we didn't have any problems. They should have updated it during his medical in Ja. So they can just transfer the info from it, just as they would the ds-3025.

Of course you just choose the state or sip that you are from in the US and then it will bring up a list of surgeons that you can phone to get you I-963a done. If you don't have the DS-3025 - you could always try and phone the doctor overseas that did it and request them to fax you a copy or mail you one. If not just try using your immunization card. Call around to get a doc that only does the I-963a and NOT the entire medical all over because you shouldn't need this and it shouldn't cost you alot. Good luck.
