Vaccinations are not a requirement to get a K-1 visa, but they are a requirement to adjust status once in the US and married. It is less of a hassle and usually cheaper to get the vaccinations before, or at the K-1 medical so that all the required vaccinations can be marked off on the DS-3025 at the medical exam overseas. Otherwise, you are going to have to find a civil surgeon in the US who won't insist on giving you another medical that is not needed.
