Yes, she does;but I've already filled ds260 (with her married name, I only realized the mistake after sending it - I was dumb enough to think it refereed to my birth, not hers)... Should I just wait?

No need to worry. NVC doesn't issue checklists for the DS-260 (as far as I know, cause in the 5 years I've been here, I've never seen one). If it's bothering you that much, just mention it at the interview, but I doubt they'll care about it. Don't bother calling NVC about it because there's nothing they can do with that form.

If 1040, then you need to put the amount in line no 22. That is the total income.

In I864, which income you mentioned? Gross income or total income? Line no 22 in the 1040 is the total income, which you need to mention in the I864.

Did you send the tax information for past three years?

In regards to the AOS form and what line to use, it's not dependent on what form you're filing (I-864, I-864A, I-864EZ). It's dependent on what tax form was filed for each year. So, for the past three years, you need to look at each tax year individually and determine what tax form was filed (1040, 1040A, 1040EZ), and put the amount based on that. Keep in mind, different tax forms may be filed from year to year (ex: 1040 may be filed one year, but the 1040EZ may be filed another year), so be careful.

Once you figure out what tax form was filed, read the instructions on the AOS form for that question as to what amount needs to be used based on what tax form was used (total income vs adjusted gross income).

And It says

[x] In Part 6. Sponsor's Income and Employment, please correct the following...

[x] Item 10. The current annual household income must equal the sum of items 5, 6.c., 7.c., 8.c., and 9.c.

I left blank all these items (5,6c,7c,8c,9c and 10)

Well, that's your issue. You need to fill out questions 5 and 10 (these are required regardless). Questions 6-9 are dependent on your case (if you have someone in your household contributing income).

Unless you are self-employed, question 5 (current individual annual income) is NOT based on tax forms, it's how much money you're making right NOW (based on pay stubs).

I have no proper answer for this. I would probably call the NVC to ask. Or just wait and if no checklist, get your mom's Birth Certificate with you to the interview and correct it then. I read it somewhere on VJ about the person who made a mistake on 260 and the NVC told them to do this. I just don't remember what kind of mistake was made. Maybe somebody else can chip in?
