My fiance received Packet 3 without the medical questionnaire also known as form IV18a.

We sent an email to the Embassy to send the form, but his medical exam is next week!

:help: Does ANYONE have a soft copy of form IV18a (Medical Questionnaire) that you can email? :help:

We are filing through the U.S. Embassy in London.

AOS - Milwaukee, Wisconsin

09-05-07 - Applied for SSN - Couldn't locate in SAVE database

09-06-07 - Mailed AOS, EAD & AP to Chicago Lockbox

09-21-07 - Received SSN in mail

09-28-07 - Received NOA I797 for AOS, EAD & AP

10-17-07 - Biometrics for AOS & EAD

11-02-07 - Received AP

11-07-07 - Received EAD

11-26-07 - AOS transfered to CSC

12-05-07 - Touched

12-28-07 - Touched

01-31-08 - Received RFE (I-693 Medical Exam)

02-21-08 - Sent RFE back to CSC

03-13-08 - Email from CSC - Case resumes

03-25-08 - Email from CSC - AOS approved and GC in production

03-29-08 - Receive GC in the mail w/ welcome letter same day

I just checked, and it's identical to mine (despite having an older date on it), except that it should have a space at the top for putting your case number. Otherwise - this is exactly it. :)

2005 - We met

2006 - Filed I-129F

2007 - K-1 issued, moved to US, completed AOS (a busy year, immigration-wise)

2009 - Conditions lifted

2010 - Will be naturalising. Buh-bye, USCIS! smile.png

Thanks you so much! I called the Knightsbridge Doctor's and they said they have a few spare. This helps me out so much. I really appreciate it!

And can I just say that my experiences with the Knightsbridge Doctors (at Basil Mansions) were excellent today. A little slow, but kept me informed and we very chatty when I was in the various rooms with them!

Does anyone know how long the medical is valid for ?

I was led to understand it is valid for 1 year.

Yep... I have my medical this afternoon and have just been re-reading through all the info & instructions... it says "the medical results will be valid for 12 months".

2005 - We met

FYI: The CIU number changes weekly, so if it's not this week's number your email may be ignored. If you call the extortionated £1.20/min visa help line, you can get a hold of the current number.
