Once the case is at santo Domingo, your fiance needs create a profil at ustraveldocs.com, then print the confirmation page with the fee to pay at Banco popular ( it is the bank use for embasdy to collect tge visa fee, 165 for k1). You dont pay online, only personal at this bank.

With the receipt number from the bank your fiance go back to the profil at ustraveldocs.com and finish the process to schedule interview at the embassy and vca appoinment.

Vca ( visa center aplication) this appoinment is before interview at the embassy, here they do fingerprints and photos taken.

with the confirmation page from ustraveldocs.com your fiance go to do medical exam , better 10 bussines days before interview at tge embassy.

Your fiance can go to medical at the morning and vca at the afternoon, in this way in one day he or she can do 2 thing in one day.

Medical exam price usually is from 280 to 350 dollars all depends from season and from the vacines the the person need.

There are tow places at Santo Domingo where do medical, people say that the new ones is cheaper that the old place.

You can rwad about the process for santo Domingo at the carebean forum, there are good post that can help you.

Good luck

If you have already scheduled an appointment for an interview, please prepare your documents as directed in your appointment letter and appear at the consulate on the appointed date and time. Otherwise, please wait until you have been notified of your interview appointment. Additional information about how Immigrant Visa interview appointments are scheduled can be found at: http://travel.state.gov/visa/immigrants/info/info_3731.html
