Photos required for medical checkup

Hi

I have medical checkup arrangement for tomorrow, they told me I need to bring 4 US visa size pictures.

I told the photographer I need US visa size, but when I check the pictures in the tool provided by US gov website, it says my head doesn't fall between two ovals. (link: http://travel.state.gov/content/visas/english/general/photos.html).

I wanted to ask how important is it at this stage (for medical checkup) to have the exact photo requirements? Or are these requirements only applicable to the pictures brought for interview?
