Fee Waiver for I-90 Renewal

Ok so first off I'm hoping this was the right place to ask this! Apologies if it isn't please direct me to the correct forum.

I haven't been at the forums in a long long time lol

Anyway, I am helping an elderly lady renew her I-90 and I noticed that there is a form I-912 Request for Fee Waiver.

Now, Section 3. Basis for Your Request Line 7. b. My household income is at or below the 150% of the Federal Poverty Guidelines...

IS SOCIAL SECURITY INCLUDED with her household income? She works part time.(I read over the instruction sheet but don't understand, perhaps I missed something. I know it is not included in the Means-Tested benefit [Line 7. a.])

Before anyone asks why isn't she a citizen please don't ask...she has been denied multiple times sadly because her English language is not that good and shes been here since 1959. She was so upset the last time she was denied she doesn't want to apply again.

I want to help her any way I can so if there is info out there please could you include that in your kind replies

Oh one more thing

for the address do I put U.S. or USA or United States for the country of residence?

I am so confused! I don't want this form coming back again for this elderly lady!
