Please help! send vacination supplement with AOS???

I was wondering if you have to send the vaccination supplement with the AOS forms, I didn't see on the instructions if you had to.

If anybody us can help in this, please do.

This is one of the questions that instigates much discussion here on VJ. :lol: You will get advice for both sending it or not.

Some have sent it and been fine, others have not sent it in with the AOS and been fine. However, some have decided to take the chance and not send the vac supp with the AOS petition and received an RFE, which can cause the AOS, EAD and AP to be delayed.

My advice! Send it in, then you know you won't get the RFE for the vac supp form.

We sent the sealed envelope we got from the civil surgeon with the AOS petition. Good luck

If you are positive that you have received all of your vaccinations prior to your medical in your home country, then it is probable that you don't need the supplement. From what has been written recently on VJ, this seems to be what is happening. My wife submitted a letter (but no vaccination supplement) indicating that she was fully vaccinated in the UK prior to arriving here and that this was stated on the medical form that was submitted in the UK. No RFE, and AOS approved with no problems.

Ks: If you were admitted to the United States as a fiance(e) (K-1), child of a fiance(e) (K-2), Spouse of a U.S. citizen (K-3), or child of K-3 (K-4), and received a medical examination prior to admission, then you do not require another medical examination as long as your application for adjustment of status (Form I-485) is filed within one year of your overseas medical examination. You will, however, be required to submit a vaccination supplement with our adjustment of status application. The vaccination supplement must be completed by a designated civil surgeon.

Rather than waste time interpreting something that is already so clearly stated, I did not hesitate to get the supplement from a Civil Surgeon in my area. And I got all the required shots in my K1 medical too (well, at least all the shots the doctor felt I needed).

The CS told me I needed the blood test for HIV, TB test, and a polio shot (even though the embassy-assigned doctor in Singapore told me I wouldn't need the polio shot). I didn't argue because it's good to be covered anyway. After all the shots were given, she gave me a copy for my records and one in a sealed envelope for my AOS application.
