immigrant data summary sheet stapled...can I remove it?

My wife has her seminar this June 10, and I read in the cfo website that she needs to bring the original and a photocopy of the immigrant summary sheet. Our problem is, the immigrant summary sheet is stapled to the yellow envelope. Can my wife remove the data sheet from the envelope? If not, what is the best way to approach this situation? Does she just have the sheet photocopied while still attached to the envelope, and then bring the whole Visa packet with her to the seminar so that they can see the original copy as well? I read somewhere here that its bad to take the data sheet from the staple. I do not know what is right. For the people who have gone through the seminar this year, please tell me how you approached the situation.

As stated on the instructions, you cannot remove the staple. So, photocopy it with the plastic on. Or remove from the plastic cover.


Do not detach from the Yellow envelope.

REQUIREMENTS

•2 valid ID's (must be a picture ID, like a driver’s license, passport, PRC or PhilPost ID. Check the website for accepted ID's collected by CFO)

•Photocopy of the 2 valid IDs (IMPORTANT NOTE: it is not indicated on the website to photocopy your ID's but, it is posted on the CFO entrance door, so photocopy your IDs to lessen the hassle)

•Passport and Photocopy of the bio page (the page that contains your picture)

•Photocopy of Visa page (visa page on your passport)

•Registration form (downloaded and printed from their website)

•Other documents: Prepare to be asked for your fiancée (petitioner’s) naturalization certificate or birth certificate and passport photocopy. Also, some are asked to present pictures of the two of you together so it's suggested bringing evidences you presented at USEM interview.

•FOR THOSE WITHOUT PASSPORT: if your passport is still with USEM, your appointment letter (with your interview schedule) will be enough in replacement of the passport and visa requirement

The DATA SUMMARY sheet is a requirement with all immigrant visas at CFO, always has been.

Do not remove the DATA SUMMARY sheet from the envelope, simply make a copy then bring everything to CFO, and like said do not let them remove the DO NOT OPEN envelope from the plastic bag (that envelope must not be torn)

  1. Original and photocopy of passport (must be valid about six months before date of travel)
  2. Original and photocopy of visa
  3. One (1) 2x2 or passport-size photograph
  4. One (1) valid identification card with photograph (eg., SSS ID, GSIS E-card, PRC ID, driver's license, postal ID, ARC, etc.)
  5. Photocopy of Immigrant Data Summary for USA-bound emigrant (must not be detached from visa packet) except for K visa holder

is the Immigrant Data summary always stapled to the yellow envelop? my mom said when she got her visa, the immigrant data summary is inside the plastic packet but not stapled on the yellow envelop.

As long as it's inside the plastic envelope then there is no concern.

The immigration summary accident detach and my flight is tomorrow...what do I need to do now I am so stress  it's accident detach because of my kid I was just gone for a minutes..when I come back to the living room my son is holding the immigration summary  but the yellow envelope pack visa it's sealed ..now my flight is tomorrow   make me nervous badly 😭😭😭😭😭😭

Thank u so much there someone advice me dont worry about my immigration summary accident detach..guys seriously   dont worry i been that..they never ask about what happened my summary data why is detach..he never ask me..kaya kayu basta pagsamasamahin but lang lang at ilagawy sa plastic  nong galing sa 2go problem...
