Packet 3 papers at RDJ-Brazil???

Hi. My fiance and I were wanting to download these documents from the RDJ-Brazil Con. Gen of the U.S. website and get a quick start on filling out packet 3 that would normally be mailed to my fiance from the RDJ office. Could you take a look at this website and tell me which of these we will need to fill out?

I'm figuring all of them but I just wanted to get some imput from someone who has already been through it. Will we need to fill out both the English and Portuguese forms that are on this list? Or, are the forms intended to be used as either one or the other? Can my fiance go ahead and choose which doctor she would like to see and go ahead and complete her medical exam? (as long as it is completed within 1 year of her interview) Can you tell me how much money my fiance should mail with her records request from the Federal and State Police? Thanks for your help.

09/2006 - Apply for K1

03/2007 - K1 approved

04/2007 - Apply for AOS & EAD

07/2007 - EAD approved

01/2008 - Conditional Residency approved

11/2009 - Apply to remove conditions

02/2010 - Permanent Residency approved

11/2010 - Apply for Citizenship

03/2011 - Citizenship approved

07/2011 - Moved back to Brazil
