I slipped up and my wife's I-94 has expired and we have not filed for AOS yet. What's the worse that could happen? What will likely happen? What problems will it cause us? Anything?

My point of view it's best to get it filed before she's been here 180 days past her visa expiration. Lots of people go waaaaaaaaaaaaaaaaaaaaaay longer than that with no problem.

From what I know, you must get married within 90 days of your entry, but you can apply to AOS after that. They say you must apply ASAP, but if they give you 90 days to get married, it makes sense that you are allowed to apply after that.

I wouldn't stress about it too much. Many others have done exactly the same thing with no problems. File the AOS as soon as you can.

We applied a month after my wife's I-94 expired and thus far have not encountered any trouble. NOA1 and biometrics appointment have both been received.

There is nothing that says you must apply. All though, who would want to live in a country with no legal status.

As long as intending immigrant entered the USA legally on a valid Visa, being out of status is usally overlooked/ Forgiven

Once you are married & AOS is filed.

That being said it would be wise to file as soon as possible

We applied about six months after and didnt have a single problem.. it was never even mentioned...

Our case was waaay messed up and my husband became out of status for like, 3 years. Don't worry about it. We had our AOS interview in Des Moines, which I take it you will, too. It was easy and the guy was pretty nice. When I was freaking out about my husband being out of status, someone here told me that is forgiven at the interview, which turned out to be the case.
