I may be being completely dence but I do know its your vaccine suppliment, is it the thing I was given with my vaccines in England? I can't figure that one out, so help the retarded person please!

No, you get the I639a at the civil surgeon in your area. You can find a list of civil surgeons on www.uscis.gov. Call them in advance. Some will want you to do the medical again but that is not necessary. All you need is the vaccination supplement I639A.

No, it is a paper given by the US civil surgeon. When you go to see CS, bring your England vaccine record or DS-2035 given by your doctor doing your physical for K1 (sometimes they don't give this to you, you have to ask him for it). And then CS will transfer the data from your England record to this I-693a form for $25 bucks (you have to call around though)

Correction: DS-3025

You should have received a copy of this DS-3025 from the panel physician when you had your medical done. A civil curgeon here in the US needs to transcribe it to a I-693a for you to submit with your I-485. We paid $10 to have David's transcribed.

Thanks Jen for the correction. Our doc in Rio didn't give us any paper. Fortunately our CS is able to transfer data from her Brazilian vaccine record without any problem.
