Are you sure? I am adjusting my status. What does the letter of intent say? My then fiance (now my husband) and I are already married.

Let me know if I am missing this.

The G325a is for the beneficiary only, with 2 passport photos, also 2 passport photos each for the AP and EAD forms if you file them.

Will have my photo taken tomorrow and expecting to receive Certifed Copy of our Marriage Contract and I think it is good to go..

Last few questions.

For I-693, since my medicals happened in February 2015, I do not need to complete this form, right? I will only have to attach the vaccination record provided to me during my medicas (I opted for this to be done, as I thought it was a requirement to obtain my K1 visa before).

For Who May File on I-765. The answer should be c9, not a6. Right?

I am changing my name, and would like to use my husband's lastname. I do not need to provide any other documents right? I only need a copy of our Certified Copy of Marriage Certificate.

Also, I would be applying for my SSN tomorrow. I only need my passport, birth certificate, ID and Copy of the Certified Copy of Marriage Certificate. I would like to have it under my new married name. Should I be taking with me additional document?

Lastly, to whom should we address the check payment?

You will need your passport and valid I-94 with at least 2-3 weeks before it expires. We had a few issues with this at our local SS office as they would only issue the card in my wife's maiden name since it matched her passport and I94 (we did have the marriage license, but it didn't matter). We ended up going back to the office two more times, once with her EAD to change her name, and once with her GC to get the "Valid for Work Only" note removed. Interestingly, it seems that going to the SS office on a Tuesday morning in Pontiac, MI is a good thing. We waited only 5 minutes before called to the window, and were out in 10 minutes. Fastest time ever.

Lastly, to whom should we address the check payment?

From I485 Instructions Page 5:

1. Make the check or moneyorder payable to U.S. Department of Homeland Security. Note: Spell out U.S. Department of Homeland Security; do not use the initials "USDHS" or "DHS"
