Confused???

Hi everyone, well we just got our certified marriage certificate in the mail and are getting ready to file for AOS. I have one question though. I am confused about the part in regards to the medical examination information. Zaeem had his medical done prior to coming here of course, he also had his vaccinations done at that time as well. Can anyone please advise me as to which form I would send with the AOS package for the medical examination. Would it be the I-693 or I-693A. Any help would be much appreciated. Thanks so much.

You need to take the vaccination supplement he got after his medical (DS 3025) and have a civil surgeon transcribe it onto an I-693A. The I-693 is the whole medical and you don't need to submit that, just the vaccination supplement.

So, do you know how I go about finding a civil surgeon who does that , is there a list or someone I can contact to get a list of civil surgeons who will do that? Thanks

Our Brazilian doctor didn't give us any (DS 3025). Our civil surgeon is able to transfer the data from her Brazilian vaccination record to I-693a without any problem. But I know some CS won't do that. You have to call around and ask them if they can do it. It is better to fax them the data first and ask them if you can do it. Our CS is $25.
