My fiance interview is in april and its impossible to schedule time to fly over there to attend the interview. I have only enough time to fly with her to the us if/when she is approved.

is it required for me to attend the interview has anyone gotten approved without attending?

No you dont need to be there during your fiance's interview.

If you think your case has some red flags, then accompanying your fiancee to her interview may help her case. Otherwise, you do not have to. I did anyway, just because I wanted to pick her up and bring her back with me.
