Hello VJ fellows,

I'm about to send my fiancé's package for I-485. Do we need to send the "medical record" (copy/ original?) to USCIS with the package? The piece of paper I have is not a medical record, but not sure what to call it. It's simply a handwriting piece of paper with shots names and marks. I believe I gave the real Medical Record to an US officer at point of entry.

if you have a copy of the DS-3025 vaccination record it doesn't hurt to send a copy, we did, although the original is in the sealed envelope. If you have a copy you can check at this site to see that it is filled out correctly, if you don't you will just have to wait and see if you receive RFE regarding the medical.

We got the RFE for medical, odds are you will have to have a civil surgeon here transcribe the records. We've been waiting almost a year though so it's not really a rush. Fun stuff AOS, you're not legal but you're not illegal. Not the best thought process in how our immigration works. Obama solved it as well as health care, he's made it magical place full of unicorns and leprechauns.
