My question is, as an American citizen, do I have to fill out 13a, in part 1 that says: My citizenship was acquired through:

If so what does this mean? It asks for certificate number and place of issuance.

Well the instructions say check one box. ..so if you check 13a no need to do 13b 13c 13d. 13adoes not ask for the things you say. .....13d does.

if you born in the usa. you will check 13a and nothing else for question 13. if you acquire your citizenship some other way you will check b or c and yes or no on d
