AOS Questions...Clarifications

I am hoping to send my AOS today but better be sure...I tried browsing for same questions but hope you don't mind if one or two of my questions have been answered....

q1: for the G325A, do I only need to send 1 copy from me (applicant) and the USC spouse (sponsor)?

q2: on the I765, which category should I put? I came on a k1 and married within the 90 days requirement and sending in EAD and AP with the AOS...©(9) or (a)(6) ?

q3: does the I864 need to be notarized?

q4: I am taking risk in sending in the vaccination record without transcription of a CS, will it be a problem ?

q5: in filling out I485, do wee need to maintain what we filled out on the DS160 before? like for example other name used? or the maiden name if the "other name used" now since I am already married to my then fiancé?

also,if I wrote an organization , should I do the same?

lastly, on the question have u ever been arrested, cited, charged....I had a dismissed case before that I disclosed(DS160), should I also do the same with the I485?

q6: is a personal check ok for the payment?

regardless of where I live, did I send my AOS right? I sent it in Chicago lockbox, with AP and EAD...and I sent it in a week after my I-94 expired....just wondering if I did it right :-(
