Forms on vaccinations ?

My fiancé had his first vaccinations abroad.

We married on Sunday he needs his second set of vaccinations before submitting his AOS.

Are there any forms that need to be filled out for this?

When the medical was completed in the beneficiarys home country and should have been given a form to show what vaccinations had been had.

Was the form checked completed or completed with possible waiver?

He only needs a first series of vaccinations. If he is going to get vaccinations records transcribed onto an I-693, the form only can filled out by civil surgeon who is appointed by USCIS. If he has completed first series of vaccination it means he does not need form of I-693 or he gets waiver. Read my post regarding my I-693 on my signature.

Okay then all you need to do is send a copy of that form with the AOS packet.

You do not need to get it transcribed onto a I-693 as the vaccinations needed are complete.

Don't stress about AOS.

It's not as difficult as it sounds.

But treat it as a completely seperate process from the k1.

This is the guide, it lists everything you need.
