Ok.. I've been reading some threads recently about Civil Surgeons and the I-693a and I've just ignored them thinking it doesn't apply to me. I thought that the vaccination supplement was the bit of paper that the medical examiner gave me in London, and to be honest I didn't even look at it, I just put it in the package and sent it off. But now I've got a suspicion that that was actually the D3025 ( ? ) something or other.

I didn't get an RFE for anything of the sort, which is a good thing. On the interview letter it says to bring the supplement if you haven't already sent it - and I guess I have.

What do you reckon I should do? There was also a vaccination on the sheet which wasn't available in england - don't tell me I need to get that done too...

I'm kind of in the same boat, but people here have advised to get a Civil Surgeon to transcribe the DS-3025 to the I-693A, just in case they say something about it at the interview.

As for your vaccinations, if you're between 18-64, you only need MMR, Td and Varicella (according to this from Packet 3).

I think lots of people get through the AOS process sending the paper the civil surgeon gave them after the medical in their own country. I think for a small fee if you call around you can have it transfered onto the I-693A by a civil surgeon .. might be a good idea to have the proper one @ the interview just in case ! As far as the vaccination i have no idea .. Good Luck !

The thing is, I didn't make a copy of the DS-3025 so I only have crappy printouts from my GP listing my vaccinations - will a civil surgeon be able to transcribe that? This is so ridiculous, what difference does it make if it's on a different form or not! I think I might not bother and just hope for the best. Distract the interviewer with my charm or something.

This IS one of the inconsistencies of AOS.

The DS-3025 is a form prepared by your panel physician during your visa medical. That physician certifies as to what vaccines he can verify you have had up to that date. It's worth noting here that it is NOT the panel physicians responsibility to see to it that you have the vaccines necessary to adjust your status once in the US - at least not in the case of those beneficiaries of a non-immigrant visa (K1 or K3).

The I-693A is an attachment to the I693, which is a US Civil Surgeon's report to our government that your vaccines are complete and in compliance with US immigration law.

The inconsistency seems to happen either at the Missouri Service Center or the AO who actually interviews you. There are many reports on VJ of AOS packets being RFE'd for lacking the I693A, even when the DS-3025 did happen to show that all the vaccines are in order. There are also many reports of VJ members sending only the DS-3025 and that being regarded as satisfactory.

I'm of the opinion that the inconsistency lies in the person looking at your file. Some may be comfortable in comparing the DS-3025 at the vaccine chart they have in their office and saying - OK this looks fine to me. Others may want a Civil Surgeon's certification because they simply don't want to be accountable.

If you've come this far, I wouldn't worry about it. If on the day of your interview your AO is not satisfied with the DS-3025, they will inform you (I've read of this once or twice). Your greencard will be delayed until you return the information. The ball is in your court.

I am in the situation as you - i sent in a copy of my Vaccination sheet from the London Embassy Doctor (from my K1) - i also have my AOS interview in LA the week after you.

As i understand it - you are *meant* to get a Civil Surgeon to transfer your vaccination info onto another sheet of paper (which you have to pay for). However, i have read many threads where people have just sent in the Vaccination sheet from London - and have been approved with no problems.

Personally - i am planning to just risk it - to take all my papers from London - and see what happens. I am guessing that if they had a problem with my AOS application they would have sent me a RFE by now.

PS not picking on your but for your educational purposes.....

Civil Surgeons are only in the US. 'Panel physician' is the correct term for the doc that did your medical prior to your interview. :blush:

I haven't decided yet whether to risk it or not. Let me know how your interviews go and i'll decide then ;). My interview's about 2 weeks after yours.

But have you had the varicella vaccine?? Wasn't available in the UK... I've had chicken pox so I can't see that having a vaccine would do any good. I'm trying to find proof that I've had chicken pox, just in case they ask at the interview...

exactly! so why doesn't the I-485 say to include " a completed 693A " in its instructions rather than the " vaccination supplement" If people dont know of the form the USCIS is setting itself up for a lot of extra work with RFE's , no? I think it may depend on the country of the beneficiary. Ive lived in 3rd world countries for the last decade and am used to the " what will you pay me to write what you want" mentality. There's nothing one cant get for a price. Its possibly a last crosscheck to see if the medical was done correctly.

Im with you, I cant find anyone around here to do it for less than $75. Im sick of it. I have a perfectly legible document. Ill just hope it does the job :yes:

I too also just sent in the worksheet. $75.00 is the cheapest around here that I could find to transcribe the worksheet onto the I693A and thats just appalling. So Im not going to bother unless I actually get a RFE for it.

Usually, the doctor accepts a verbal statement that you've had chicken pox. If not, get a blood titer test done checking for Varicella antibodies to prove that you have had chicken pox.

P.S. You can also get a test done checking for MMR antibodies, but not Td--you will have to get this redone if you have no proof of vaccination.

You can always go to the interview and if they want the I963a then just say that you had sent the ONLY copy of your DS-3025 with the aos application and the immigration officer for a copy of it. Then you can get it transcribed and your off to the races. Hopefully they are nice enough to give you a copy of it.

to the original poster: I also had the chickenpox, and I had to take the blood test to prove that I had it.

Though, I didn't care at all, I just went there, gave them my arm and said "Do what you gotta do."

They did, and after a few days i got my forms. It only cost me like 150$ or so.

Filing for N-400 now
