Timeline

AOS packaet readey. Upon review I discovered that the receipt for his vaccinations from his medical examination is in Spanish!! Although the names of the shots are in the English the document is in Spanish and the payment stamp with date is in Spanish. Should this have been translated into English also!!!!! :ranting::crying::crying::crying:

Wait, what you need is the vaccination documentation worksheet, that's the DS-3025 which is in english, if you have that then you're good.

They sent all of his medical documents straight to the Embassy and the Embassy did not give him any documents. He only has the receipt that he paid for vaccines and the names of the vaccines that he paid for. How do I get a copy of the DS-3025?

He was not able to pick up his medical results so they were sent directly to the embassy. They did not give him any medical docs. No x-rays, no CD, nothing!!! I guess they put them in the sealed envelope. The only thing that he has in his possession is the receipt that he paid for the vaccines and the name of each vaccine.

He just spoke with the Consultorios de Visa in Santo Domingo, the name of person on the receipt and her number, and they said he was supposed to ask for it, which he didn't and they didn't give it to him. Now they are telling him, that he has to wait until Monday to find out exactly what happened!!!! :shocked::shocked:

Is it possible for me to have this record transferred from this receipt to the DS-3025 by another doctor, here in the United States, or does he have to get these vaccines again here in the United States :girlwerewolf2xn: ?

Wait so the DS 3025 apply to CR1 cases too? Who do you have to ask back for it?

What if I don't have a DS-3025?

If you are positive that you got the needed shots, then actually mailing in a DS-3025 photocopy is not necessary. The original form is with your other medical results and was turned over to USCIS at POE. That's actually the one that counts because it remained in the "chain of custody" so you didn't have an opportunity to alter it. The photocopy isn't really an official copy and is more for a reminder that you got complete at your visa exam. Of course without having a copy, you don't have the opportunity to see if the rest of the form was completed correctly.

I would include a copy of the vaccinations receipt and a statement with it that says his DS-3025 was turned in at his POE along with the rest of his case files and medical documents in his sealed envelope.

Ok if he got approved and his passport was stamped with the K1, then the embassy should had gotten his complete medical and the DS-3025, and was in the sealed envelope like you said, hopefully! My experience with the consultorios de visa was good, wait until you have an answer from the consultorios de visa.

If you don't have a document that says he actually got the vaccines, not just pay for them, he may have to do the whole thing again, maybe; the thing is can he get the vaccines again in short period of time? Ask the consultorio de visas if they can send him the DS-3025, which is what's needed for the AOS at the moment.

Just in case, get that receipt translated because it proves he already paid for the medical and vaccines.

The DS-3025 is included with the rest of the medical documents in your case file, which was in the sealed envelope you turn over at your POE. The DS-3025 is not really a requirement to send along with the I-485. Some countries do not even bother to give vaccinations or provide a properly completed DS-3025. People include a copy of the DS-3025 to show that they had all the required vaccinations and are not in need of seeing a civil surgeon for an I-693.

Wait so the DS 3025 apply to CR1 cases too? Who do you have to ask back for it?

It should but I'm not completely sure since I was not there with him during the process and I can't really tell you what all transpired during the medical. My information is second-hand through him, but if these guys on this site says it does, tell your fiance to ask questions about getting a copy of all the medical documents that he will need to adjust status here in the states.

Hopefully someone who actually went through the medical will enlighten us more.

Update: Consultorios de Visa in Santo Domingo emailed a copy of his DS 3025. All was in order. AOS documents mailed on 13 Feb 2015. Thanks to all for your help and input.

Now the waits begins again. :thumbs:
