AOS Packet, Name Change question. (merged threads)

Hi guys. This is my DS-3025. I'm 24 years old and got confused about my medical. I just remember I didn't get a tetanus vaccine and other one but forgot what was it and the doctor told me that I could get it here in US. We're gathering the requirements for AOS and don't want to get RFE. What is/are my missing vaccines that I need to get. Please help. Thank you.

my K1 and K2 are both missing vaccine (varicella). They did not have it in the Philippines when they had their medical exam. Where should they get vaccinated? Civil Surgeon or local pharmacy? If we get it from a local pharmacy, we still need to go to the Civil Surgeon right?

You can get the varicella vaccinations anywhere they are available: hospital, clinic, pharmacy, etc. You will need to bring your DS-3025 AND the proof of the varicella vaccination to a civil surgeon to have those vaccinations transcribed onto an I-693 form. Submit the I-693 with your AOS application.

You can get the varicella vaccinations anywhere they are available: hospital, clinic, pharmacy, etc. You will need to bring your DS-3025 AND the proof of the varicella vaccination to a civil surgeon to have those vaccinations transcribed onto an I-693 form. Submit the I-693 with your AOS application.

Thanks Zedayn! I guess we can just go to civil surgeon for vaccine since we are going there to have it transcribed anyway.

So you'll gonna pay twice if you get your missing shot/s lets say at Walgreens. The shots and the civil surgeon service fee for transcribing the DS3025 to I693?

Yes, but you would pay a separate fee for the shots from a civil surgeon as well. Vaccinations are not included in the price to transcribe the vaccinations onto an I-693.

Are those supporting documents like Marriage certificate needs to be certified copy? Because we are already submitting certified copy for the Adjustment of status. Thanks

Yes it is the same address. One is the physical address and the other is the mailing address. Both go to the same location. You will use the express mail address if you are using an express mail service, such as FedEx, UPS, or even USPS express mail.
