Hello, I need to ask the community for some help to understand something. I am trying to determine if my wife needs to see a civil surgeon. From what I am understanding from the instruction sheet for the I-693 form she does not need to see one. She came over on a K1 visa and we were married within 90 days, she had the medical exam done by a civil surgeon as part of the visa process and we received a copy of the DS-3025 Vaccination record, which I understand was filed with all her other paperwork when she was approved for her visa. Do we still need to have a civil surgeon fill out page 1 of the I-693 or does she even need to fill that one out.

Thanks for the help.

Yes, the civil surgeon needs to transcribe the info from the DS-3025 over onto the applicable parts of the 693 form. Do a Google search to find the search engine for CSes in your area. Call them first and inquire about their fee to transcribe only (be careful so that they don't try to redo the complete physical medical) as described. They all charge whatever fee they want, so call around.

No.

You do not need to see a civil surgeon IF she has the box checked completed vaccinations on the form she was given.

You only need to see a civil surgeon if she does not have all the vaccines required at the time for AOS.

The only reason she would need to then is because you would have to add any new vaccinations on.

Check if it is completed.

If her vaccination record or DS-3025 is completed or the waiver blanket is ticked then she does not needed I-693. Check my signature, I wrote my experience regarding this.

Not an accurate response...

If the during the medical exam all the vaccinations were provided or identified as contraindicated or not age appropriate on the DS-3025 then there is no need to see a civil surgeon when applying for the AOS.

If you had all the required vaccinations marked off on a properly filled out DS-3025, then no need for a civil surgeon. See this post for details> http://www.visajourney.com/forums/topic/428381-i-693-report-of-medical-examination-and-vaccination-report/?p=6206232
