Really confused about DS 3025 and form I-693

Hey everybody my wife and I are about to send AOS papers and stumbled upon form 3025 which states that she is missing varicella shot and that applicant may be eligible for blanket waiver because vaccination is not medically appropriate so do we need to print out form I-693 and find who the civil surgeon is ? Or just send the form 3025? My wife came here on a K1 visa we got married in May and currently live in Delaware,US

If I were you, just send a copy of her DS-3025. That's what I did. Read my experience on my signature. My gamble was, even though I got RFE, the process is already very slow especially to those who came here with K-1 visa. So far, I did not get RFE and got interview waiver letter and am still waiting for my I-485 to get approval.

There are a lot posts regarding missing NOA-2 hardcopy. Based on from what I have read, shouldn't be a problem because logically she can't have K-1 visa without I-129F had been approved.it also says you have to pay $400 for replacement which I think is really outrageous. Did you see an approval of I-129F on USCIS? Just print it and send along with the package including the explanation about missing NOA-2 hardcopy.

We did not go a CS and submitted the DS 3025 along with these instructions: Green Card in hand with no RFE!

3. What if I am a K nonimmigrant visa holder and already had a medical exam overseas?

If you were admitted as a:

a. K-1 fiancé(e) or a K-2 child of a K-1 fiancé(e); or as a

b. K-3 spouse of a U.S. citizen or a K-4 child of a K-3 spouse of a U.S. citizen; and

c. You received a medical examination prior to admission, then:

(1) You are not required to have another medical examination as long as your Form I-485 is filed within one year of your overseas medical examination, and the medical examination did not reveal a Class A medical condition, or if you did have a Class A medical condition, you received a waiver of inadmissibility and have complied with the terms and conditions on the waiver.

(2) Even if a new medical examination is not required, you must still show proof that you complied with the vaccination requirements. If the vaccination record (DS 3025) was not properly completed and included as part of the original overseas medical examination report, you will have to have the vaccination report completed by a designated civil surgeon. In this case, you are required to submit the vaccination record and page 1 of Form I-693.

I am actually in the exact same boat.

I came to the US a month ago, and I am applying for AOS now.

I have read other posts on this site where people have written "get the DS-3025 transcribed by a panel physician, have them fill vaccine part of the 1-693"

Is that what you are doing? Or is that outdated information?

I called one of the panel doctors, and was told the whole thing would cost $125.. so I am so confused, as others said the transcribing would cost $15-50.

Is there a specific thing we need to ask the doctor to do?
