Hi everybody good day to all, my wife just received an email from embassy in manila stating that she needs to get a NBI will an Aka that she never used she inquired about at the NBI office close to her house was told and i quote we don't issue with a name never used by applicant, so what do we tell the embassy at her interview on February 12th, i will be there with her i will just tell them what was told to her since her old passport was in her maiden name only till she changed to her married name.and the name that they're talking about is her died husband whom she never used his name on nothing

First don't tell NBI that you never used that name. THEN complete the form including "the never used name" as the AKA. Git er dun!

NBI CLEARANCE (Green Form): Applicants aged 16 years and older must have a valid Record Clearance (for travel abroad purposes) from the National Bureau of Investigation (NBI). Clearances should be in the applicants current name, birth certificate name, maiden name, married name, and any aliases or nicknames ever used, including different spellings of all names ever used. An official letter of explanation from the NBI is required for any notation of No criminal record, or "No pending case. For immigration purposes, an NBI clearance is considered valid only for one year from the date it is issued. You may contact NBI at (632) 523-82-31 to 38. If she got married to her dead husband and it was a legal marriage with a marriage certificate, it counts as a name even if she never personally used it.

On the NBI Clearance application form there is a space for AKA/other names used. What the NBI said is technically true. They'll issue her a clearance in her current name but they'll run a check on all other names she'll list in the AKA. That check for all names is what the Embassy is looking for. Follow what Hank said and you'll save yourselves time because odds are you'll get a 221g and have to do this anyway.

In the interests of helping out, I physically redacted my wife's NBI Clearance of personally identifying Info just to show you what we did. Click on the attachment at the bottom of this post.

As you can see, she requested for the Clearance in her current name (the one her passport, license, etc shows) and in the request form we added multiple AKA versions of her name. So in sum, the NBI Clearance shows the following:

Jennifer - Mother's Maiden - Maiden; with Her Married name under "Husband's Surname"

AKAs:

Jennifer - Maiden

Jennifer - Married

Jennifer - Maiden - Married

(Just as an aside, I was definitely rejoicing when I saw the "No Derogatory Record" note when we got her Clearance)

To add my 2 pesos ... The more recent the NBI the better. Definitely anything older than 6 months is seriously pushing the limit for a 221g, which recently happened to a friend of ours. It's less than 200 pesos (less than $5 USD) to get a new one. Some places are harder than others to get one. We got one in olongapo in less than 30 minutes.

Thanks manilaraf now my wife knows what to do you saved the day. I mean the embassy asked for my 2014 w-2 too that was the easy part cuz i already have it ready, plus they ask for cenomar to which she already has her previous husband's death certificate, I'm just glad they told us about what they need to see so i will keep everyone posted on my wife's interview so everyone thanks a lot for sharing

Your wife will have more to fill out since she has to put her former married name as well as her current married name. Just remind her to put it all on there. Better to have too many names on the NBI Clearance than to have too few.
