Do I need I-693??

Hi All! I have some questions about I-693.

By the way, I'm here now at California (thanks for your help guys) and I'm already married to my fiance. I now preparing documents needed for my adjustment of status and I have a questions about form I-693. I had my medical exam at St. Luke's last November 17th-18th and I got two shots, one for Tdap and one for MMR. As far as I remember the doctor told me that I should have 3 shots but the other one is not available that time. When I reviewed my DS-3025 (Vaccination Documentation Worksheet), the Vaccine under Varicella has a legend letter "D" (not routinely available) and Td has a legend letter "B" (Insufficient time interval to complete series). Do I need to get those shots and see a civil surgeon and get I-693 before applying for my adjustment of status?? My DS-3025 says "US Vaccination requirements COMPLETE (Requesting a Blanket Waiver), what does it mean..?

I'm worried and of course I don't want to get RFE.

Also I checked the Form I-693 in USCIS website and the form says Expires 01/31/2015. Where can I obtain a new one? Thanks!

If you are getting the form form the USCIS website then it is the most recent version. The office of management and budget (OMB) is responsible for updating the forms departments of the US government use. They have a process that everone must follow and part of that process is that the form "expires" a certain amount of time after it is submitted to the OMB. The OMB puts that date on the form. It is up to the department that uses that form to either update it or accept it after it expires. So this form has expired, but the USCIS is still accepting it because they have yet to get around to updating it.

From the USCIS website regarding the I-693 form:

Edition Date

01/15/13. A new edition of this form is coming soon. In the meantime, customers may file using the 01/15/13 or 10/11/11 editions.

If your vaccination record was complete then you only need to submit the DS-3025.

Varicella or a history of chickenpox is required for AOS.

Hmmmm... I never had a chickenpox... So I guess I need to see a civil surgeon, bring my DS-3025 with me along with Form I-693... Do i need to pay for the vaccine?

You have to pay the civil surgeon and whatever vaccine required.

If you have health insurance coverage, you might want to check if the vaccine is covered by the insurance.

If you don't mind me asking, what did you end up doing?? My vaccine paperwork also says "complete" but I'm debating on if I should get the i-693 filled out by the civil surgeon or not.
