I know I should ask a Civil Surgeon these questions, but I'm still trying to find one who doesn't want to charge me for an office visit to look at my papers and tell me what I need.

I just got my wife's Vaccination Documentation Worksheet sent from Cambodia and it says her vaccinations are incomplete.

Also attached was "Appendix C. Vaccines To Complete for Adults".

Under "Td (Primary Series)" which I think is Tetanus, it says she needs "dose #2, 4-8 weeks after dose #1" (this is indicated by a handwritten arrow pointing to the checkbox for Dose# 2).

Dose #1 was done May 26, 2006.

It also has a checkbox for "Dose #3, 6-12 months after dose #2", and a checkbox for "Booster done every 10 years after first 3 doses". These have no handwritten arrow.

My questions are:

Since it has been so long, does she have to start over with Dose #1?

and

Is Dose #3 required, which would mean waiting six more months to file?

If anyone has any experience with this, thanks for your help.

I only had to have one tenanus shot done. That schedule sounds like the one for Hep B, which my daughter had to have. Prolly best to consult a doctor on this one.

Td is tetanus+diptheria.

Td involves 3 doses over 8-12 months.
