We're ready to file for my wife's AOS (she came in on a K1 visa), but we wanted to make sure whether or not we still need to see a Civil Surgeon to transcribe a I-693.

My wife had her medical done in Manila, Philippines and received all the vaccines except TDAP which was not available at the time. I could have her take a TDAP vaccine at our local public health clinic and get a vaccination card or something, but I was wondering if we can file the AOS using just the DS-3025 she received from her medical exam.

Attached is her DS-3025... do we still need a I-693?

I think you still need to do I-693 since her vaccine is incomplete on DS-3025. You can have TDAP at any cheap clinic and have the civil surgeon just transcribe it.

Btw, need civil surgeon too if ever she has class B (latent TB) medical.

AOS Journey:

March 30 - AOS/EAD/AP sent

April 2 - USCIS received

April 9 - NOA1

May 6 - Biometrics

May 7 - Case is ready for interview

May 21 - Request for EAD expedite

May 26 - contact congressman for EAD expedite

June 1 - EAD card in production

June 2 - EAD approved

June 6 - EAD/AP combo card received

You will need to go to a CS and have them complete the vaccination history portion and either give her the required shots or state they are not age appropiate. Only if the box that the vaccination record was complete would allow you to skip the I-693 and use the DS-3025

You will need to get the Tdap shot as that is missing from the DS-3025. You can get the shot where ever is convenient or cheapest for you, and then take your DS-3025 and proof of your Tdap shot to a civil surgeon and just have them transcribed onto the I-693.
