It seems like everyone else get their DS-3025 form oversea from their doctor but not my wife!

I talked to my wife and she said when she did her medical in Rio centro, she didn't receive any form from the doctor. The doctor just talked to her and transferred the vaccination data from her Brazil vaccination card to whatever forms he was writing on and sealed everything in the mysterious brown envelop. SHE DIDN"T GET ANYTHING FROM HIM??????

We just called the Dr. and the secretary said that they didn't keep any record of this type of visit. So how else do we get that DS-3025?

We're (potentially) in the same boat. If your wife had all her vaccinations before coming to the U.S., you can try writing a note that says that her full vaccination record was submitted at the POE. Some people get RFEs for that, but some don't. I called the help line, and an officer told me that the vaccination supplement was really only meant for those who didn't have all the vaccinations available in their home country.

We have a (second) RFE on the way now, and I'm worried that it may be for the vaccination supplement. My husband does have his vaccination record from his childhood doctor, so we may see if one of the civil surgeons will accept that. Alternatively, he'll have to have the whole medical exam again. :rolleyes:

Conditions Removed! Ten-year green card received 6/24/2009. No more USCIS for a very long time.

My hubby & I are in the same boat!!! I just rec'd a letter from the doc who performed his medical. It states that his chest x-ray was normal, he tested negative for HIV & Syphillis, and that his vaccination history was complete(prior to exam).

I am going to try to get a doctor here to fill out the I-693, and hope for the best.

It is ridiculous that he paid $320 Euro & can't get a copy of the ds-3025, but that's the way it goes! Good luck to you

My fiance is actually on his way here to the US :dance: But I remember telling him to get a copy of the medical records when he had them done in Poland... the doctor said they don't give them out, everything goes into the Brown envelope used at POE. I guess it's just another way to suck out some money from us, and re-do the medical part :(

Just like the Doctor did in the foreign country, you should be able to take the vaccination card and give it to a civil surgeon here in the US for him/her to transfer the vaccination information onto the I-693. All the doctor did on the medical form was transfer the information provided already. You don't need your "medical" that was part of the package that was given at the POE.

Thanks for all your input. I am going to call the Civil Surgeon and ask them about the transfer from her Brazil Vaccination card to the I-693a. I feel like I am being scammed.

The doctor we used in Rio also did not give us the document or keep the records. Civil surgeon in the US will complete a vaccination supplement for you. Shop around. Some of the Civil surgeons charge way too much.

I simply called the doctor in Canada and asked for a faxed copy of the form that was sent to the Consulate. The office faxed the DS-3025 minutes later. It is outrageous that the doctor would not have a copy of the medical record on file. Doctors are suppose to keep medical records on their patients!

So you use her Brazilian vaccination card instead?

The civil surgeon said that they could not read the vaccination card (not English), and wanted to test her for vaccines. I check with our insurance, and they paid for tests and vaccines. Then, civil surgeon filled out vaccination supplement 693 for a small fee.
