question #14: National Identification Number? ...what are they asking for?

question #22: when do you intend to arrive in the US? ...can we write "when visa is granted"? how? or do we have to provide a specific date? or can we leave it blank?

question #20: list below all places you have lived for at least 6 months....i lived in ontario for just a month...do i still have to write it or no need?

question #22: list all employment for the last ten years? ...do we still have to put the unemployed or just the employed ones?

question #14: National Identification Number? Some countries have a mandatory ID system like: China, (Canada and the USA do not have a national ID card). You can put "NONE" there.

Identity (ID) cards are in use, in one form or another in numerous countries around the world. The type of card, its function, and its integrity vary enormously. Around a hundred countries have official, compulsory, national IDs that are used for a variety of purposes. Many developed countries, however, do not have such a card. Amongst these are the United States, Canada, New Zealand, Australia, Ireland, the Nordic countries and Sweden. Those that do have such a card include Germany, France, Belgium, Greece, Luxembourg, Portugal and Spain.

question #20 list below all places you have lived for at least 6 months. Yes The at least 6 months mostly refers to "Countries" if in same country and in a different place you can note it.

question #22: list all employment for the last ten years? You should list the periods of unemployment.
