We just recently got married this past December and currently in the process of completing the I485 package.

I need help with the I-693. We currently have on hand a lung Xray CD, yellow immnuzation booklet and paper work showing what shots have been done already. Since my wife has had her health examination overseas which was done less than a year, she is not required to do it again I believe.

She recently has taken the 2nd doses of her shots (varicella, MMR, etc.) recently here in the states, but it was not done by a civil surgeon. It was done at a local pharmacy/clinic and paper work was provided by the clinic which showed proof of getting the shots done.

My question is:

1) now do I have to find a civil surgeon who is willing to just do the paper work and put it in a sealed envelope? The clinic said this was possible and many of their patients have done it this way.

2) I keep hearing about the DS-3025, should she had received it in her home country? I believe my wife received a packet and gave it to the immgration officer at POE.

3) Since we were married last December and her entry stamp on her passport expires the end of this month, there should be no rush since we're already married correct?

Yes, you can get the shots from anywhere but only a USCIS approved Civil Surgeon can complete the vaccination supplement for you, so schedule an appointment with such and provide the vaccination record in advance and they should be able to determine what vaccinations, if any, are needed. If vaccinations needed, get those anywhere or at Civil Surgeon. Then visit the Civil Surgeon for completing the vaccination supplement form and obviously pay for that service. They will then hand you the completed form as per USCIS requirements which you submit to the USCIS along with the 485. The DS-3025 is downloadable from the internet. Link below. You can also use the bottom part of the I-693 form, same vaccination supplement doc. Civil Surgeon should also have such forms at their office.

You only need one in a series for AOS, so you would not need to see a civil surgeon for an I-693 just because you had follow-up doses.

Don't fill out and use your own DS-3025 or I-693. The DS-3025 is provided by the panel physician at your K-1 medical, and the I-693 needs to be filled out and signed by a civil surgeon, if you need one.

Some countries do not give a copy of the DS-3025, but it is included and in your file with your other medical forms you turned over at your POE.

Ok, I need someone to clarify, because one comment says to get the Civil Surgeon to fill out the vaccination section of the i-693, and if so, I'm assuming the other sections won't need to be filled out because my wife already had the health examination done within one year?

the other comment says I may not need to see a Civil Surgeon for I-693.

One question that I need to get answered is that her Visa show her entry time will be expiring the end of this month which was stamped on her passport when she arrived here on the K1. Since we're already married, we shouldn't need to worry about the expiring date that was stamped on her passport on point of entry correct?

I'm still not understanding....?

He does have the DS 3025 the panel physician checked off vaccine history incomplete AND applicant maybe eligible for blanket waivers because vaccinations not medically appropriate.

So what does that mean. ? Can we just send a copy of that or we need to find a civil surgeon to sign off on a I693?

If so where can I find the list of civil surgeons?
