Dwheels I don't mean to be a bother but I went on the link and I still did not see the medical history form could u be a bit more specific?

And that is where I read that u need to register your appointment? Which u didn't answer whether I needed to?

And if I live extremely close to the consulate do I still need to register with DHL?

when u get the P4 email print out the attachment twice, set your medical appointment and take your vaccination record and P4 to medical -no history form-. take the other P4 to interview with evidence you want to take. you must register your appointment to get your visa package when approved.

Hi mrs m. Had the same problem. Wondering where to get the medical form. Called andrews memorial to enquire about it and was told to expect in the mail along with my interview letter or I could just purchase one on the day of the medical appointment.

the doctor's office provides the form. all you need for medical is vaccination record and P4 letter.

My CC was on 04 Feb 2015, and it ship out on 05 Feb, is this mean they scheduled the interview already?????

CC on 04 feb 2015.my document shipped same day because i called on 04 feb the lady tald me it just right now your case completed,

next day i went to DHL.com and tried this; tracking by reference

EXP 04 FEB 2015A

range bewteen 03 FEB 2015 and 05 FEB 2015

country Saudi arabia ( i believe only one case to saudi arabia approved )

BOOM showed me all step from Boston to Bahrain in middell east right now

you got it !! , if your CC on 04 FEb then i believe your shipment of 05 is yours.

Got my CC on Feb 4th and my papers are already in belguim. They'll get to the destination Tuesday at the latest...this is all too exciting!

Thanks,I check it myself and saw 2 cases as well how do I know if 1 of them is my case should I call NVC on Monday to verify?
