I'm now filing I-485 to adjust my status from K-1 however I am lacking the DS 3025 vaccination records. I have completed everything before my K-1 interview. Unfortunately, the immigration officer at point of entry did not return any documents to me. I am not sure of any ways to get a copy of that form so I'm planning to write an explanation and let them check that in my personal records instead, do you think that will work? I am just wondering if you have any idea on how to resolve this?

Thank you so much! This is the only document that I am lacking. I'm hoping to send the documents as soon as possible.

If you are positive you had all the required vaccinations at the time of your K-1 exam, then you can add a note stating that your vaccinations are included with the medical files you handed over at your POE upon entry. It is not a guarantee of not getting an RFE, but I would try that first and wait to see if they ever want anything more from you.

Thank you KayDeeCee for the prompt response! Yes, I am positive so I am now thinking of attaching my vaccination booklet as a supplement and all the laboratory results from my medical exam. It has all the information they need except it is not written as neatly as in DS 3025. I will send my documents now so as not to delay any further. i'm hoping not to get an RFE though 'coz I really have no idea where to get DS 3025. I've written to the clinic though to see if they can send a scanned copy to me but there is a little chance to that so I am not really expecting a reply. Thanks again!! :)
