Sample format for Cover Page of AOS?

Hi to all,

I am preparing for my AOS application and just like to ask some help on how to create my Cover page.

I hope you can give me format or sample of your cover page.

Your help is much appreciated.

Copies of documents submitted are exact photocopies of unaltered documents and I understand that I may be required to submit original documents to an Immigration or Consular officer at a later date.

Instead of sending money order(s), you can send a check for the mentioned amount, in the description I wrote I-485 filing fee + biometrics fee for XY. Also, you need to include two passport style photos for EAD and AP (2 for each), with your name and A# written on the back.

Also, I'd add 'with supporting documents' after listing I-765 and I-131 because you need to send the biographical page of your passport + I-94 for both of these too, as they are processed separately from your I-485.

And on top of everything put G-1145 if you wish to receive email notifications when they receive your applications and also list it in the cover letter.

Hi,

i didn't send any cover page. They don't need, they want all the forms and checks money, but if you have particular advice about your application and have to explain something, so yes it is necessary.

You do how you feel, if you prefer put cover page, so do it.
